An Auto Planning Workflow On Pinnacle Planning System for Cervical Cancer

Purpose: An auto planning workflow was established on Pinnacle planning system. Plan quality and planning efficiency of auto plans generated by this workflow were compared with manual plans.
Methods: 10 patients with cervical cancer were enrolled in this study. Intensity-modulated radiotherapy (IMRT) plans were manually generated by a dosimetrist. An auto planning workflow was established by Pinnacle scripts and Python codes. All planning workflow including re-optimization could be completed by this auto planning workflow except organ at risk (OAR) contouring. And constraints in optimization process also could be auto adjusted according the real calculated value. Plan quality and efficiency were evaluated by both dose/volume indices and Equivalent uniform dose (EUD) – equivalent uniform distance (EUL) metric between manual plans and auto plans.
Results: Auto plans reduced mean dose of bladder (Dmean = 33.25 Gy) compared with manual plans (Dmean = 34.72 Gy) with significant difference (p = 0.01). However, there were no significant differences observed in other dose/volume indices of these two plans. The planning time of auto plans was dramatically reduced compared with manual plans (auto plans: t = 15.90 min, manual plans: t = 66.90 min with p < 0.01). For EUD – EUL evaluation, auto plans spared more bladder with higher consistency (R2 = 0.8966) than manual plans (R2 = 0.7039). And these two plans produced comparable rectum radiation dose with similar consistency.
Conclusion: Auto plans generated by auto planning workflow could dramatically improve the efficiency of planning, while with comparable plan quality compared with manual plans. Furthermore, plan consistency of auto plans was better than manual plans for bladder.
