Purpose: In present study, we analyzed the variation rules of volume and morphology of heart, pericardium, and the left ventricular muscle during cardiac cycle based on ECG gated heart 4DCT during breath holding.

Methods: In present study,we analyzed ECG gated heart 4DCT images of 14 patients during breath holding. And the 4DCT was sort into 20 phases according cardiac circle. Heart, pericardium, and the left ventricular muscle were contoured on each phase CT images. The volumes variation in different phase were analyzed, and the morphology change was studied refer to the first phase.

Results: (1) The time resolution of each phase CT could reach to 40ms. (2) The maximum change of volume were 16.3±5.4%, 13.3±5.0%, and 27.0±16.3% for heart, pericardium, and the muscle of left ventricular in cardiac cycle. (3)The mean DSC(Dice’s Similarity Coefficient) were 0.88±0.02, 0.97±0.01 for heart and pericardium in different phases refer to the first phase.(4)The mean DSC of the muscle of left ventricular in different phases was only 0.61±0.21(0.11-0.94), and the mean variation rate reached to 240±200%, which was larger than those in heart and pericardium significantly (P<0.05).

Conclusion: The volume variation of heart and pericardium in cardiac cycle were consistent, which were less than 20%. The morphology change was less than volume change for heart and pericardium, however, the morphology change of left ventricular muscle was more significant than volume, which was also more significant than those of heart and pericardium.
