Is It Safe to Contour Pericardium Instead of Heart in Radiation Therapy of Thoracic Tumors?

Purpose: The purpose of this study was to analyze the volume variation of heart and pericardium following cardiac cycle based on electrocardiogram (ECG) gated heart 4DCT during breath holding.
Methods: In present study, ECG gated heart 4DCT images of 16 patients during breath holding were analyzed retrospectively. Each CT was sort into 20 phases according to cardiac cycles. And the heart and pericardium were contoured on each phase CT images respectively. The variation rules of volume of heart and pericardium were subsequently analyzed, and the volume of heart in different phase beyond the maximum and minimum pericardium volume were record and calculated.
Results: The maximum volume of heart and pericardium were mean 609±119cc and 715±150cc respectively, and the minimum volume were mean 525±103cc and 632±130cc. And the largest volume of heart in different phase beyond the maximum pericardium was mean 4.71±2.13cc (0.86±0.43%). The biggest volume of different phase heart beyond the minimum pericardium was mean 52.98±22.00cc (8.78±3.25%).
Conclusion: The heart was not be covered completely by pericardium contour in some phases of cardiac cycles, the heart dose evaluation based on pericardium contour which was determined on static 3D planning CT would cause some errors.
