Purpose: Dosimetric comparison of Co-60 and Ir-192 HDR source used in brachytherapy treatment of cervical cancer.

Methods: The computed tomography (CT) scans were taken for 15 patients suffered with carcinoma of uterine cervix using 3 mm slice thickness. The contouring of HRCTV, bladder and rectum on CT images were done as per GEC ESTRO guidelines with the help of MRI images in treatment planning system (TPS). All parameters were kept same for Co 60 ( 3.5mm active length, 0.5mm active dia, bebig) and Ir 192 ( 3.5mm active length, 0.6mm active dia, bebig) HDR source with 2.5 mm step size and dose prescription was done on Point A. Volumetric parameters D90 D100, V90,V100,V200,V400 were calculated for HRCTV and D0.1cc ,D1cc ,D2cc ,D5cc ,ICRU reference points for bladder and rectum.

Results: Study shows that the D90 , D100, V90, V100, V200 and V400 values of HRCTV in Co-60 are higher by 7.02%, 7.56%, 0.67%, 0.90%, 4.69% and 9.43%, respectively as compared to Ir-192 source. The difference in dose values at D0.1cc, D1cc, D2cc, D5cc and ICRU reference points of bladder are 1.5% ,0.16%, 0.36%, -1.11% and 0.17%, respectively. On the other hand, dose difference for D0.1cc ,D1cc ,D2cc ,D5cc ,ICRU reference points of rectum are 1.54%, 1.54%, 1.52%, 1.91% and 5.82%, respectively.

Conclusion: Present results show that all the dose parameters of HRCTV, bladder and rectum are higher for Co-60 as compared to Ir-192 HDR source except D5cc of bladder. The isodose distribution is more bulge out in cranial caudal direction. The clinical evaluation in each slice and plane is necessary to explore the advantages of Co-60 source over Ir-192 HDR source.
