Comparison of GYN Brachytherapy Treatments Using Tandem-Ovoid Applicator, for Co-60, and Ir-192 HDR Brachytherapy Sources

Purpose: Treatment of cervical cancer cancer patients may become more convenient with high dose rate (HDR) Co-60 brachytherapy source instead of HDR Ir-192 , The aim of this study is to compare the dosimetric aspects of the GYN treatments as well as the treatment time with cobalt and iridium sources for treatments of cervical cancer using Tandem-ovoid applicator.

Methods: In this study, Ir-192 source model mHDR-v2r and BEBIG Co-60 source model Co0.A86 were simulated using MCNP5 Monte Carlo code. The dose to points A, and B were determined from treatment planning data of a patient treated with Tandem-ovoid applicator. Identical dwell positions and dose to Point A were used for both cases. With this arrangements, dose to point A, DVH for Bladder, and rectum as well as the total treatment times have been compared between the two cases. The TG-43 algorithm was used for dosimetry. Then, the treatment time was obtained for different activities of Co-60, and 192-Ir such that the dose to points A, and B is fixed. Obtained treatment time for two sources according to its activities was compared with each other.

Results: According to the results, the treatment time for cobalt source according to half-life and dose distribution point A, B points is shorter than iridium source. The results of this study indicate that the treatment time necessary for treatment of the patient with a cobalt source having 2.13 Ci activity is almost the same as an iridium source with 5.68 Ci activity.

Conclusion: According to the results, Ir-192 can be effectively replaced by higher half life Cobalt-60 in GYN brachytherapy. Our results indicates that the Co-60 HDR system can provide the same prescription dose for the GYN implants with improved DVH to bladder and rectum.
