Development of a Quality Assurance Software Tool for a New Tomotherapy Machine

Purpose: To ensure treatment quality and accuracy with a newly-marketed tomotherapy machine, Radixactâ„¢ (Accuray Inc.), which features fully integrated planning, delivery and data management, we aim to develop a robust quality assurance (QA) tool to automatically and independently check data integrity, plan quality, beam-on time, and treatment delivery.

Methods: A software QA tool, TomoQA, was developed to retrieve data in the Radixact system via its gateway configured to report plan and delivery summary. The plan and delivery parameters are parsed from the exported xml files and are compared automatically. A nomogram was developed to check any gross error on beam-on time as a function of various machine and patient factors including target site/size, fractional dose, modulation factor, and jaw width. Plan quality is evaluated by comparing the achieved dose-volume data with planning goals. The treatment data in both Radixact system and our main record and verify system, MOSAIQ, are also compared and verified.

Results: The nomogram created based on data from prostate patients, treated so far with Radixact in our clinic, shows that a linear fit on the relationship of beam-on time and the machine and patient factors yields an R2 of 0.99, indicating the feasibility of identifying gross errors on beam-on time using TomoQA. The tool successfully checks treatment planning and delivery parameters in two stages: (1) data integrity and plan quality, including consistency of demographics in Radixact and MOSAIQ, the correctness of images, structures, mass density model, machine parameters, dose optimization and calculation parameters, plan transfer from Radixact to MOSAIQ, and (2) consistency of the planned and delivered parameters, as well as the appropriateness of patient positioning shifts.

Conclusion: The TomoQA software tool can effectively and independently check the entire planning and delivery on the highly-integrated Radixact system and has been used in our routine clinic.
