As 3D imaging (CT, MRI) is adopted for brachytherapy planning, real-time planning and optimization of dose using inverse-planning algorithms is becoming feasible. As a result, implementation of HDR interstitial brachytherapy for gynecologic malignancies has undergone considerable technical and workflow changes. The aim of this session is to summarize the current state of image-guided interstitial HDR brachytherapy planning and to compare and contrast different loading and planning techniques.
A panel discussion of interstitial GYN brachytherapy treatments will be presented whereby each speaker will pose a series of questions to the panel and each panelist will present their institution’s practice. Topics to be discussed include 1) applicator geometry selection and digitization, 2) use of MRI, and 3) planning strategies with a focus on the impact of increasing complexity on treatment delivery safety and workflow efficiency. This format will enable a variety of interstitial HDR workflows for treatment and planning to be presented to the audience while allowing them to draw their own conclusions about the extent to which increasing complexity is appropriate for their clinic.
Learning Objectives:
1. Describe the treatment and planning workflow for interstitial HDR brachytherapy for gynecologic malignancies
2. Discuss the role of 3D imaging including CT and MRI for interstitial HDR planning
3. Compare/contrast the use of standard loading to dosimetric optimization for plan development
4. Understand the impact of increasing complexity on QA and safety
