If you are paying by check, please make the check payable to AAPM. Please mail your check and the registrant's name and copy of registration document to:

Invoices
If you will need an invoice prepared to pay for your registration, please send your request to janet@aapm.org Please note that if you have registered at the early rate, we must receive the payment on or before January 17th. Otherwise you registration will be automatically changed to reflect the late registration fee. All payments must be received by February 1.

Cancellation Policy
Fees for registration will be refunded in full if written notice of cancellation is received by Friday, February 1, 2013. Please send your cancellation in writing to Rachel Smiroldo or fax to 301-209-0862. No refund will be given for cancellations received after February 1.
On-site Registration Days and Hours
AAPM staff are available to assist you during the following hours.
Registration desk located in the DoubleTree Dallas Campbell Centre lobby, 8250 North Central Expressway, Dallas, TX:
- Thursday, February 7: 5:00pm-8:00pm
Registration desk located in the Texas Health Resources University building:
- Friday, February 8: 7:00am-5:30pm
- Saturday, February 9: 7:00am-1:30pm
