Fees for registration will be refunded in full if written notice of cancellation is received by June 25, 2007. No refund will be given for cancellations received after June 25. Please send your written cancellation and refund request to Hadijah Robertson at hadijah@aapm.org or fax your request to 301-209-0862 (Attn: Hadijah).
If you have registered at the discounted rate and are paying by check, your payment must be received no later than June 1, 2007 (11:59 PM EST). If we do not receive your payment for the discounted registration fees by June 1, your registration type will be changed to reflect the non-discounted registration fee.

How Do I Register for SAMs sessions/credits at the 2007 Annual Meeting?
AAPM wishes to take any steps required to ensure that no individual with a disability is excluded, denied services, segregated or otherwise treated differently due to the absence of auxiliary aids and services identified in the Americans with Disabilities Act. Companions are not eligible for MPCEC. Companions ages 12 through 17 will be allowed in the Exhibit Hall but must be accompanied by a registered attendee at all times. Children 11 and under will not be allowed in the Exhibit Hall or session rooms.

MOC/SAMs
SELF ASSESSMENT MODULES

Completion of two Self Assessment Modules (SAMs) per year is a requirement for the Maintenance of Certification (MOC) process as defined by the American Board of Radiology (ABR). AAPM program organizers have identified sessions in Diagnostic, Nuclear Medicine and Therapy Physics to be offered with audience response technology at the annual meeting in 2007.

Junior Member Weekly	This registration type is reserved for Junior Members of the AAPM only.
Non-member Resident Weekly	In order to qualify for this registration type, you must fax a letter from your Program Director that includes: the name of the institution and its address phone, fax & e-mail of the program director area of specialization (e.g., Radiation Oncology Physics, Radiation Oncology, etc.) start date and intended completion date Failure to fax this letter will render your registration incomplete.
