- Fees for registration will be refunded in full if written notice
of cancellation is received by July 6, 2005. No refund will be given
for cancellations received after July 6. Please send your written
cancellation and refund request to Shantelle Corado at shantelle@aapm.org
or fax your request to 301-209-0862 (Attn: Shantelle).
- If you have registered at the discounted rate and are paying by
check, your payment must be received no later than June 8, 2005. If
we do not receive your payment for the discounted registration fees
by June 8, your registration type will be changed to reflect the non-discounted
registration fee.

from your Program Director that includes:
- the name of the institution and its address
- phone, fax & e-mail of the program director
- area of specialization (e.g., Radiation Oncology Physics,
Radiation Oncology, etc.)
- start date and intended completion date
Failure to fax this letter
will render your registration incomplete.
