One of the most important tasks that therapy physicists perform is the review of plans and charts prior to and during treatment. This is a key quality control check and is one of the most effective measures for preventing mistreatments and near misses. However, there are no recommendations for best practices of plan and chart review and very few related reports can be found in the literature. The purpose of AAPM Task Group 275 is to provide recommendations for effective practices of both the physics initial and weekly chart checks. These recommendations are established by combining an AAPM all-member survey of current review practices with a structured assessment of potential risk using a systematic FMEA-based risk methodology. The Task Group report presents best practice recommendations for physics plan and chart review in external beam therapy (photons, electrons and protons) as well as HDR brachytherapy for gynecological applications.
