NRC proposes a civil penalty of $39,000 against the Department of Veteran's Affairs for two violations of NRC regulations.
The NRC identified that the DVA failed to recognize that problems with prostate brachytherapy programs existed at other VA facilities after similar problems were previously cited at the Philadelphia VA Medical Center's program.
